Sometimes it is just a simple lack of spreading out the petitions. I visited a very large Lutheran church at which they made intercession for somewhere near 100 people. When I asked why they didn't spread them out amongst their four weekend services, they looked at me as if it had never occurred to them at all. (I had a professional relationship with two of the pastors, to the point at which I felt I could be candid.)
